Binaural beats: what the evidence actually says

Binaural beats are created by playing two slightly different tones in each ear — say 200 Hz in the left, 210 Hz in the right. Your brain doesn't hear two tones; it perceives a single pulsing beat at the difference between them, in this example 10 Hz.
The theory, often called brainwave entrainment, is that this perceived beat frequency encourages your brain's electrical activity to synchronize toward it. A 10 Hz beat, for instance, sits in the alpha range associated with relaxed wakefulness.
The evidence here is genuinely mixed. Some controlled studies have found modest reductions in self-reported anxiety with binaural beats in the delta and theta ranges, particularly in clinical or pre-procedure settings. Other trials have found no measurable difference versus a matched control sound, and effect sizes across the literature vary widely, which is often a sign that individual differences, expectation, and study design matter a great deal.
What most researchers agree on is that binaural beats require stereo headphones to work as designed, since the effect depends on each ear receiving a distinct tone. Listening through a single speaker collapses the two tones together and removes the mechanism entirely.
A fair, current summary: binaural beats are a low-risk, inexpensive tool with plausible support for mild relaxation in some people, but they are not a validated treatment for anxiety, insomnia, or any clinical condition, and shouldn't replace care from a qualified professional when one is needed.
